Caseworker II – Home Based Services
Community based Caseworker II available as part of our Home Based Services program. Caseworker II will work with children and adolescents receiving services through Park Center’s programs. Caseworkers have the primary responsibility to coordinate and facilitate services in order to ensure continuity of care by:
- Conducting parenting education
- Providing goal directed services
- Providing strengths based services, and
- Communicating progress and problems with physicians and/or therapists.
Applicants must have a bachelor’s degree in a mental health field with one year experience or master’s degree in a mental health field with six months experience. Valid driver’s license required. Discretionary bonus available.
