Park Center offers a range of mental health services designed to assist and support the psychiatrically disabled individual transition out of homelessness into a stable environment in the community.  Homeless Team Liaisons provide services to people without stable housing who have serious and persistent mental illness.  The team focuses on individuals who are admitted for Park Center services and are considered homeless, with the understanding that their needs are complex and multifaceted.  Attention is focused on the following:

 

  • Assistance with locating housing by linking with shelters, halfway houses, and other resources to obtain housing.
  • Addressing the individual's mental illness and encouraging the individual to remain engaged in treatment - with stability being the main goal.
  • Case management and other support services available to those who choose to receive Park Center services.
  • Outreach, education' and engagement at local shelters of those who are in need of the team's assistance.
