ADULT COMMUNITY SERVICES
Mental illnesses are equal opportunity diseases that can happen to anyone at
any time. The effects can be debilitating and can sometimes limit a person’s
ability to function in daily life. Severe and persistent mental illnesses also
impact one’s ability to live an independent life, which most of us take for
granted. At Park Center, we offer comprehensive and individualized treatment
services in the least restrictive setting, helping people readjust to the
community and lead independent lives.
The primary goal of Adult Community Services is to stabilize adult clients
experiencing acute psychiatric reactions. Other goals are to monitor and support
chronically mentally ill persons and to serve those clients who can benefit from
outcome-focused, time-limited, rehabilitative services. Case management services
are provided; linking resources, medical, and professional treatment with the
goal of promoting self-sufficiency. The continuum of services may include:
Hospital/Inpatient

Park Center contracts with a local hospital for inpatient services. The focus
is on acute, short-term inpatient treatment. Park Center medical staff provide
treatment, and clients are actively linked with Park Center’s services to
enable a successful and quick return to the community. Hospital liaison staff
are responsible for the case management of the client’s care through the state
and local hospital system. Court liaison staff assists clients with the varying
needs concerning psychiatric and legal issues, such as involuntary
hospitalization and guardianship.
Transitional
Care Services - (260) 969-8440
Transitional Care Services (TCS) is a crisis
stabilization program. TCS is a 24-hour, 30 bed, sub-acute, transitional
residential unit designed to assist those with serious mental illness and/or
substance addicted clients in remaining in the least restrictive environment
during episodes of psychiatric decompensation. With the help of annual grants
from the Parkview Community Benefit Program, this service promotes a strong
continuum of mental health services in the community.
TCS is designed to
prevent reduce hospitalizations and prevent further decompensation. It
also provides respite placement for family caregivers, temporary shelter for
mentally ill homeless adults, and temporary placement for clients discharged
form state operated facilities. Emergency services are also coordinated as
part of this program.

Adult
Residential & Supported Living Services - (260) 481-2850
Varying levels of residential and supported living services are offered to meet the needs of
persons who are diagnosed with a chronic mental illness with varying levels of
severity. The goal of Residential Services is encouragement of individuals to
function as independently as possible in the community.
Park Center offers a range of housing opportunities that complement
therapeutic needs. Below are the residential options serving acute needs, to
less structured, more independent options.

Case
Management Services
Park Center’s Case Management Teams offer two levels of case
management services:
Assertive Community Treatment Team (ACT)

The ACT team is designed to work with individuals who
experience significant disability from their mental illness. Often, these
individuals are resistant to treatment, and are unable or unwilling to
participate in office-based services. They may be high users of acute services
and may have co-existing substance abuse or criminal justice involvement.
Continuous Treatment Teams: (CTT)

The Continuous Treatment Teams primarily work with individuals who have
reached some level of stability in the community, but still require continuous
treatment and monitoring.

Outpatient
Stabilzation - (260) 481-2750
Outpatient Stabilization (Day Treatment) offers the advantages of an
intensive treatment program while allowing an individual to maintain their life
in the community. The goals of this program are to promote stabilization of the
acute crisis as well as the chronic elements of mental illness. Within
this program, there is a specific treatment methodology for individuals
diagnosed with Borderline Personality Disorder. Dialectical Behavior Therapy (DBT)
is a specialized program that teaches self-containment skills and uses specific
protocols for individual and group therapy.

Drop-In Center

The Drop-In Center offers socialization, educational, and prevocational
activities which are continually updated to meet the needs of clients. Classes
are goal oriented and are designed to help improve community living skills. The
Drop-In Center offers a friendly, supportive environment where clients can
interact on an informal basis. Clients are encouraged to take an active role in
the daily operation of the facility, and the running of classes and activities.

Eagle
Employment
Services - (260) 481-2878
Eagle Employment Services, a Park Center supported employment
program, provides vocational services to adults who are diagnosed with a mental
illness. This service locates the type of employment that best matches both the
applicant’s and employer’s needs. Eagle Employment offers the benefit of a
job coach to work with and train the employee on the job, as well as monitor the
employee’s job progress, if needed. Staff work closely with employers to
guarantee satisfaction with the employee.
